Bandera Healthcare, LLC was the owner of Park Avenue Health and Rehabilitation Center in Pima County as of the first quarter of 2026, data from the Centers for Medicare and Medicaid Services (CMS) shows. The company has held ownership since Jan. 30, 2006.

The facility operates as a for-profit nursing home.

Park Avenue Health and Rehabilitation Center received an overall CMS rating of four during the period, surpassing the Arizona state nursing home average of 3.4. These ratings assess facilities based on health inspections, staffing, and resident care quality.

CMS data shows that in the first quarter of 2026, Park Avenue Health and Rehabilitation Center averaged 144.6 residents per day, with 200 available beds. The nursing home is not affiliated with a hospital.

During the same period, Park Avenue Health and Rehabilitation Center paid one fine totaling $25,116 and received one penalty.

Estimates from the U.S. Census Bureau indicate the older adult population is rising nationwide as younger populations decline. From 2020 to 2024, Americans 65 and older grew by 13%, while the population under 18 shrank by 1.7%. The country’s older adult population has risen sharply over the past century, climbing from 4.9 million in 1920 to over 61 million by 2024.

A 2021 study found that nearly half of older Americans—around 28 million people—could need long-term services and supports. More than a third of this group is expected to eventually utilize nursing home care.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an insurance education group. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
